Heritage Health’s history is the basis for and really drives our mission and organization as a whole. We started as a free, volunteer run clinic one evening per week to deliver medical services to the underserved in our community. Even though Heritage has morphed over the years and rebranded, our mission is still the same: to deliver a healthcare experience that provides hope, inspires change, and extends life for our patients and our community.
We started originally with primary care and dental services to meet the needs of our most vulnerable in the community. We now also have mental health support with family support services (our counseling team), psych services, and restored paths (our addiction services team). Our post acute care team is based within Skill Nursing Facilities and Assisted Living Facilities in the community. Finally, we have a street medicine community outreach team that is partly mobile, partly clinic based and meets the homeless within the community and treating them where they are. All of our teams work together to give our patients wraparound, all encompassing healthcare.

Family Medicine Residency of Idaho Nampa is a new training program recruiting for an experienced physician to join our faculty group. We are developing a 6-6-6 Family Medicine program in a Teaching Health Center and small community hospital. Our program will focus on preparing residents for rural practice and care of the underserved. We are looking for a physician interested in broad scope in-patient work with adults, OB (ability to do CS if fellowship trained) and adolescent care. Outpatient work will be in a Federally Qualified Teaching Health Center (FQHC) that is also a level 3 PCMH. We are supported by a clinical care team of a psychologist, dietician, clinical pharmacist, care coordinators and community health workers. We believe in team-based, patient focused care and community engagement. Our community is approximately 35% Spanish speaking and so Spanish language skills are appreciated. Boise VA Medical Center, Boise – is seeking 1 primary care physician to add to their expanding Telehealth Program. The Boise VA serves patients in locations throughout Idaho, Washington, Oregon, and Alaska via telehealth. Physicians will work on the Boise VA Medical Center campus and serve patients at VA clinics throughout the Northwest using state-of-the-art Telehealth equipment and the assistance of a nurse at the patient site. Cascade Medical Center – Seeking Family Physician – We are looking are looking for a physician interested in practicing a broad scope of clinic and hospital based medicine. Cascade Medical Center is a Rural Health Clinic and Critical Access Hospital, operating out of single building in a beautiful mountain setting. Our facility sits a block from Cascade Lake in beautiful Valley County, Idaho less than 2 hours from Boise. We are located in an outdoor enthusiast’s paradise with year-round recreational activities right outside out door. As a provider here, you will see patient in the clinic, treat people in the ER and care for them in the hospital. Our Rural Health Clinic is open 5 days per week and we see patients on both a scheduled and walk-in basis. Clinic is open 8:30am to 5:30pm M – S and 1-5 Sunday. Each provider sees between 8 and 15 patients per day in the clinic. We do full-scope primary care – preventative care, well and sick; pediatrics to geriatrics; and acute to chronic. The hospital treats an average of 4 patients per 24 hour period in the ER and we have an average inpatient census of 1.
We do not do OB or surgery.
We have a full lab, CT, X-ray and ultrasound. Our LCSW provides counseling and social work services in an integrated behavioral health model. We have physical therapy on site as well.
